Health Systems Strengthening (HSS)

Health Systems Strengthening (HSS)

Our Programs & Interventions

We use a hybrid operational model that blends technical assistance with direct service delivery support. This enables us not only to advise governments on strategies but also to work alongside them to implement, test, and scale innovative interventions in real-world contexts.
Our PHC systems-strengthening initiative has contributed to enhanced management capacity in over 807 health facilities across seven WHO domains: Human Resources, Supply Chain, Data Management, Financial Management, Quality Management, Facility Management, and Community Relations. By embedding performance monitoring tools and promoting routine data use, we ensure facilities are accountable, efficient, and responsive to community needs.
We also play a convening role by bringing together governments, donors, and partners to coordinate investments, avoid duplication, and amplify impact. Through this, we’ve mobilized resources, improved alignment, and built stronger ownership at sub national level.

Our Programs & Interventions

Measurable results that demonstrate our commitment to lasting change

807 facilities strengthened, with measurable improvements across all seven PHC management domains

65%+ improvement in facility performance scores, contributing to better quality of care

$260,000 in counterpart funding mobilized from state governments, showing commitment and sustainability

Institutionalization of routine data-driven decision-making, improving transparency and accountability

Nigeria’s PHC is the first point of contact for most citizens, yet it suffers from chronic underinvestment, weak management, and inconsistent service quality. By addressing these systemic gaps, TAConnect ensures that every intervention including MNH, immunization and family planning rests on a strong, sustainable foundation.
