Across Nigeria, PHC M&E systems have long faced challenges such as weak technical capacity, fragmented governance, inadequate financing, and limited data use. These gaps undermine accountability and evidence-based planning, making it difficult for states to track health outcomes effectively.
Programme Overview
Monitoring and Evaluation (M&E) have proven to be a solid accountability tool that helps objectively evaluate the success of interventions. This project assessed PHC M&E systems in three states to identify weaknesses and chart a path toward stronger, more sustainable M&E systems.
- Programme Goal
Conduct formative assessment of the PHC M&E system, identify existing gaps, and understand current policies, capacities, and resources to inform an evidence-based PHC M&E Systems Strengthening Plan (PMSP).

- Utilized Measure Evaluation M&E Capacity Assessment Tool (MECAT)
- Analysis of twelve key M&E domains
- Mixed-method assessment approach combining qualitative and quantitative methods
- Stakeholder engagement across state, LGA, and facility levels
- Development of tailored, costed M&E strengthening plans
